Glomerular flltration represents one of the basic mechanisms in the function of an organism. Our understanding of this process is still quite fragmentary. Regulation of blood flow and pressure, together with regulation of the ultraflltration coefficient (which is an attribute of the flltration barrier), are the two fundamental mechanisms accounting for maintenance and adaptability of glomerular flltration. However, its widespread use during the past 20 years has enabled clinicopathological correlations to be made and the natural history of many 'glomerulopathies' to be established, though large areas of uncer­ tainty remain. In contrast to symptomatic measures, such as the management of uraemia or the nephrottc syndrome, the use of treatment in arresting the progress of the glomerular lesion itself is controver­ sial and is based on anecdotal evidence rather than controlled trials. Knowledge of the renal pathology will, at least, enable conditions in which immunosuppressive therapy is appropriate to be distinguished from those which are benign or self-limiting. In this book symptomatic management of the clinical syn­ dromes is detailed, but classification is firmly based on renal histological appearances. The relevance of immunofluorescent microscopy, ultrathin section examination and electron micros­ copy to our understanding of pathogenesis is described, as well as the uses of these techniques for morphological discrimination.

Knowledge of the juxtaglomerular apparatus (JGA) of the kidney and of the synthesis and secretion of renin has increased to such an extent over the past few years that it is now appropriate to summarize this knowledge in a monograph on the JGA, the first of its kind. It was the authors' special concern to demonstrate the association between structure and function for renin secretion, not only within the juxtaglomerular region, but also in the region of the renal cortex beyond the JGA. Perhaps a compromise position would be to make a few general statements and pose a limited number of questions which hopefully would then be addressed dur­ ing the formal or informal portions of the meeting. The goals and objectives of this conference, as set forth by its organizers, Professors Ponti celli, D'Amico and Minetti, are rather simple and straight­ forward; namely, to review and elucidate the immunopathophysiology of cryoimmunoglobulins and auto-antibodies to immunoglobulins (lg) and, secondarily, to explore the possible participation of these disordered states in glomerular injury. As such, this conference is principally devoted to an analysis of two properties of certain species of the globulin fraction of serum proteins, specifically the ability to self associate on the basis of immune interactions and to form insoluble aggregates when exposed to reduced ambient temperature. As we shall see, these two properties are often dis­ tinctly related.
